The Centers for Medicare and Medicaid Services said it has accepted 150 applicants to join the Advancing Chronic Care with Effective, Scalable Solutions program, which aligns Medicare payment with measurable health outcomes.
The agency's Innovation Center launched ACCESS to expand access to technology-supported care for obesity, diabetes, musculoskeletal pain and depression patients on Original Medicare.
CMS said in Monday's announcement that it is also extending the application deadline to May 15.
Care organizations are expected to provide integrated, technology-enabled services, including clinician consultations, behavioral support, diagnostics, medication management and medical device monitoring.
The voluntary payment model directs care to conditions affecting more than two-thirds of people with Medicare, CMS said.
ACCESS focuses on four clinical tracks:
- Early cardio-kidney-metabolic conditions (eCKM)
- Cardio-kidney-metabolic conditions (CKM)
- Musculoskeletal conditions (MSK)
- Behavioral health conditions (BH)
CMS said that private payers, representing 165 million members across Medicare Advantage, Medicaid and commercial coverage, have also committed to aligning with the ACCESS Model's payment approach.
"ACCESS introduces a way of paying for care that focuses on results," Dr. Oz said in the December program announcement. "It offers clinicians a new predictable payment option, giving them the flexibility to use digital tools that help people take charge of their health."
Most of the organizations have not previously served Medicare beneficiaries, CMS said. Participating organizations must enroll in Medicare Part B, meet licensure, HIPAA and U.S. Food and Drug Administration requirements.
Through the program, care can be provided in person, virtually or asynchronously, but participating providers must appoint a physician clinical director to oversee clinical quality.
CMS said it is still reviewing applications, which are due by May 15. Applications received after this date will be considered for a Jan. 1, 2027, start date.
All accepted organizations named below, which may be participating in one or more of the four ACCESS tracks, must enroll in Medicare Part B as providers or suppliers and comply with quality and licensure requirements, data privacy and security standards, and report outcomes.
