CAPG taps Health Access Solutions to help with coordinated care

The California Association of Physicians Groups (CAPG) has enlisted Health Access Solutions to help with its accountable care efforts – bringing the company's web-based multidisciplinary care coordination platform to more than 150 physician groups and nearly 15 million patients.

Recent healthcare reform legislation, such as the Patient Protection and Affordable Care Act (PPACA), highlights the critical need for a patient-centric, multidisciplinary delivery system to coordinate care while balancing quality, utilization of services and cost.

Patient-centered medical home (PCMH) and accountable care organization (ACO) models are outlined within PPACA as effective methods to achieve this degree of collaboration – as is the need to develop effective healthcare IT to support these forward-looking strategies.
 
Executives from CAPG – which is co-producing the National ACO Congress that's being held through Oct. 27 in Los Angeles – say the organization is committed to improving healthcare in California (and nationwide) by way of the ACO-style care environment.

“Certainly, there is no ‘cookie-cutter’ approach to structuring an ACO,” said Don Crane, president and CEO of CAPG. “But the goal of the Health Access Solutions and CAPG partnership is to support healthcare organizations as they strive to achieve genuinely coordinated care.”
 
He added that the breadth of functionality available through Health Access Solutions is vital for care coordination. “Electronic health records and health information exchanges have brought us part of the way, but multidisciplinary, collaborative technology is needed to meet the demands of new delivery models born from healthcare reform legislation. The Health Access Solutions patient-centric platform comprehensively addresses the needs of the provider, payer and, most importantly, the patient. It is designed to facilitate clinical integration and ensure the patient is at the center of all care.”

The multidisciplinary care coordination platform from Health Access Solutions – which includes both case and utilization management components with Access Express, and clinical functionality with Excelicare – "has been designed specifically to help providers monitor, treat, track and report on every patient at every point of service in the acute, ambulatory or home setting," he added. "The volume of information that will be available along the care continuum can only help improve both care and efficiency."
