California has selected Innovaccer to compile aggregate claims data from 16 participating managed care plans, representing approximately two million of the state's Medicaid population, the company said this week.
The project will equip Medi-CAL providers with actionable intelligence to close care gaps and improve outcomes, according to the company.
WHY IT MATTERS
The California Equity and Practice Transforming Health Program (EPT Program) will use the new population health analytics data to enable its statewide PopHealth Learning Center to track quality management and performance for Healthcare Effectiveness Data and Information Set reporting to the U.S. Centers for Medicare & Medicaid Services and other compliance and operational needs.
Innovaccer said its Gravitycloud platform will generate insights for safety net providers' advanced HEDIS dashboards, enabling practices to track and improve their quality performance metrics and enhancing care coordination for the vulnerable populations they serve.
The platform will also streamline quality reporting for EPT Program compliance and support efforts to secure payments tied to performance metrics, the company said Tuesday.
Dr. Jennifer Sayles, the state Learning Center's chief executive, said primary care practices across California will gain "unprecedented visibility into performance" with access to a single, aggregated score across plans and performance metrics and cited the importance of this under the "state’s ambitious transformation" plan.
It will help providers meet the state's program requirements, "earn payments and improve the lives of millions of Medi-Cal patients," she said in the announcement.
THE LARGER TREND
In February, Innovaccer announced that the Sacramento County Department of Health Services began using its population health analytics platform to exchange data across health, criminal justice and social services sectors.
The goal was to improve care coordination, delivery and access to critical programs for Medi-Cal and justice-involved populations.
The new health information exchange enhanced the county public health department's mission, according to Tim Lutz, the agency's director.
"By integrating data in real time, we can enhance care coordination, streamline services and ensure individuals have better access to the resources they need to recover, stay healthy and build resilience," he said in a statement at the time.
